2009-2010 School Year Insurance

The NC State Sports Medicine Department has sent out the 2009-2010 school year student-athlete insurance packets, for both incoming and returning student-athletes.  Please complete all the forms, with a front and back copy of health, dental, and prescription cards, and return them in the self-addressed stamped envelope by May 22, 2009.  If you have any questions or have not received your packet by Friday, May 15, 2009 please call Glenda May at (919) 515-3065.
