NCSU Sports Medicine requires all new student-athletes, returning student-athletes and walk-ons to complete medical forms and return them to the Sports Medicine Department before participating in any team related activity.  Clicking on the appropriate link below will direct you to all the forms required.  All the forms are in PDF format.  Print and complete each form then send the completed form to the following address:
